Church Questionnaire Information

Why fill it out?

Yes, it asks a lot of questions. We interviewed Board members, committee heads, and staff including the minister to see what information they would find useful to know. As examples, our board president has asked for a list of people who checked a number of specific skills the stewardship campaign will need. The Share and Care committee needs to know who will take food, write cards, drive, etc. What classes should be offered in the future? Your requests will dictate this.

Online directory information:

You will not be listed unless you give permission (on the Family sheet.) Only basic information will be listed: address, phone(s), and email. Children will not be listed. The only access to the directory is through the website with a randomly-generated individual password sent to you by the Task Force in the near future.

Getting the PDF questionnaire to the church office:

You cannot save the PDF document on your computer after filling it out but you can print it. Here are the options:

1. Print it out and place in the labeled box in the office hallway.
2. Print it, scan it, or email office@uucbham.org.
3. Print it and fax to the office at 205.945.8111.
